The Health Foundation of Greater Cincinnati conducts a number of regional surveys, including the Greater Cincinnati Community Health Status (every five years; next in 2010), the Greater Cincinnati Child Well-Being survey (on the same schedule), The Ohio Health Issues Poll (annually), the Kentucky Health Issues Poll (annually), various minority health surveys, and others.
If you have a suggestion of a topic to include in one of our surveys, please fill out this form. Our Director, Health Data Improvement Program, will review the suggestions and select topics to include. There is no guarantee that we will be able to use the suggested topics. We will concentrate on topics that can be benchmarked against national or state level data so that we can compare how our region is doing.
We ask for your e-mail and phone number in case we need to find out more information about the topic you want to include and to let you know the status of your request. We will not use this information for any other purpose.
