In response to feedback from grantees, the Health Foundation has gathered examples of outstanding grant documentation from some of our past projects. These examples include documents from all stages of our grantmaking process, from letters of intent to final reports. Whenever possible, the documents are included in their entirety. However, in order to maintain confidentiality, some information has been removed.

We recently made changes to our process and outcome evaluation plans which are not reflected in these examples. For example, process evaluation plans now require annual rather than quarterly targets. Also, some projects, such as those that are part of the Regional Primary Care Access Initiative (RPCAI), have special reporting requirements. As grantees submit new plans that include these changes, we will post examples of the newer materials.
