This grant provides a part-time salary of $30,000 a year for two years and $14,500 for technical assistance starting in January of 2000. Formerly, this was an all-volunteer agency. The purpose of this grant is to make fundraising from the private sector available to many more non-profit health care provider agencies, which are usually understaffed and under-funded and often do not have the resources to envision alternative solutions to the problems they face.

The grant of $74,500 has enabled The Health Initiative to develop a five-year strategic plan, which includes state of the art computer hardware and software to streamline the research phase of grant writing. It also includes the development of organizational procedures and public relations materials to facilitate both outreach to provider agencies and successful grant writing. The grant will also fund the development of volunteer training materials with the goal of providing more grant writers to more agencies in need of technical assistance.

Results

The number of patients served continues to report at a consistent level on an annual basis.  Health outcomes have been difficult to capture; however, it is expected that with the model of a mobile medical team, homeless persons are less likely to excessively use more costly services (e.g. emergency room).  Persons are hopefully able to better connect for other services to address issues of social isolation, substance abuse and chronic mental illness.

To provide expansion space for program improvements at the Pike Street clinic. This site is affiliated with the Northern Kentucky Family Health Network.

Results

Renovation was completed September 1998; an open house was held on Nov. 9, 1998. The expansion has resulted in the availability of three exam rooms, treatment room, storage and office space.

The project was delayed in an attempt to use ChoiceCare Foundation funds as a match for HUD money that would consolidate other services at the same site. When this was determined not to be possible, the project proceeded without it.
