Results In April 2002, the School-Based Health Center of St. Bernard-Elmwood Place Schools opened. Utilizing the services of a nurse-practitioner, case manager, and health aide, the program provides behavioral and physical health care to children from pre-school to eighth grade at the district’s middle school and two elementary schools. The spring focus is on connecting with the families of incoming preschool and kindergarten children and familiarizing families with the full range of services the center offers. The outcomes we hope to achieve are to:
  • increase the availability of and access to primary health services for low-income children through the delivery of high quality healthcare services on-site at Elmwood Place Elementary,
  • increase the awareness of and utilization of existing health and human service providers, and
  • provide on-going assistance to children in securing a “medical home” where they can receive services from a physician and staff who are familiar with them and their needs.
